No more paper forms, manual data entry, and scattered documents. With the new Forms feature, each appointment can automatically generate the document you need — filled out, sent, and archived in seconds.

Why we designed this feature

At agenda.ch, we have a long history of working with healthcare professionals, therapists, coaches, and municipal administrations. One observation kept recurring: managing documents related to appointments remains a significant point of friction — for both professionals and their clients.

A patient manually filling out a health form in the waiting room. A secretary transcribing the information into the software. A municipality having to draw up a rental agreement for each room reservation. These are repetitive, time-consuming, and error-prone tasks—which add no value.

The Forms feature was designed to eliminate precisely these frictions.

What the Forms feature allows

Create custom forms. Design your own forms with the fields you need: administrative information, health questionnaires, terms of use, contractual clauses. Each professional defines their own template.

Send via SMS with one click. From the patient's or user's file, send a secure link via SMS. The recipient replies directly from their phone, without needing to install an app or create an account.

Data integrated into the file: Responses are automatically saved in the corresponding file on agenda.ch. No manual entry, no risk of loss or transcription error.

Automatic document generation: Based on the data in the file and the answers to the form, agenda.ch automatically generates the final document: consent form, rental agreement, tracking sheet. Ready to archive or have signed.

Two concrete examples of use

Therapy Practice: Upon arrival, or before your appointment, you will receive a link via SMS or email requesting you to complete a health questionnaire. Once your answers are received, the consent form is automatically generated and added to your patient file.

Municipal Administration: When booking a room via agenda.ch, a single click is all it takes to generate a pre-filled rental agreement with the applicant's information, the reserved time slot, and the applicable terms and conditions. The document is then sent by email for signature.

An integration approach, not an additional tool

The strength of the Forms feature lies in its native integration with agenda.ch. The forms are not an external module: they draw directly from the data in your files, reservations, and calendar. There is nothing to synchronize, nothing to export.

Each piece of information collected enriches the existing file. Each generated document is automatically archived in the correct location. Your team's workflow remains intact—simply accelerated.

Note: Forms are available for all active agenda.ch accounts. Initial setup—creating form templates and configuring generated documents—is done directly from your administration interface.
