We've been working on this for a while and we're finding it hard to contain our enthusiasm... Digital forms are coming to agenda.ch, and we sincerely believe it will change your daily life.

The problem we want to solve

You probably experience this every day. A new patient/client walks in, and they're handed an A4 sheet of paper and a ballpoint pen that only works half the time. At the doctor's office, it's their medical history and allergies. At the beautician's, it's the client file with contraindications. At the personal trainer's, it's the health questionnaire. And every time, someone has to re-enter all of that into the system. Honestly, in 2026, we can do better!

With our digital forms, the client fills everything out either from home , before even arriving, or on-site . On their phone, tablet, or computer. The information goes directly to their profile on agenda.ch. Zero copying, zero paper lost in a pile.

Not just for customer records

This is where it gets really interesting. These forms can be used for everything: service contracts , consent forms , terms and conditions, medical history forms... The client reads, signs on the screen, and it's neatly archived. No more scanning, no more overflowing binders.

We know that each profession has its own specific needs. A physiotherapist doesn't require the same documents as a beauty salon or a nutritionist. A physiotherapist needs a consent form for their treatments, while a therapist needs a health questionnaire. That's why everything is customizable: you create your templates, choose the fields, and associate them with your appointment types.

What makes us proud

Beyond the technology, it's the human impact that motivates us. The reception process becomes smoother, whether in a doctor's office, a hair salon, or a wellness studio. The professional already has the essential information when the client arrives. And the client feels taken seriously from the very first contact. It's a small thing, but it changes the atmosphere.

Next steps
